Advanced Investigating and Managing Patient Safety Events

Course Details
  • This course is open to anyone who has completed the Introductory Investigating and Managing Patient Safety Events course 
  • A broad focus on various aspects of managing the occurrence of harm, and patient safety events as well as SSA:PSR methodology
  • A practical, case base 2½ day course 
  • Participants are expected to complete a patient safety review as a project for the course and mentors will be provided 
  • Participants will be eligible for continuing education credits (College of Family Physicians | Royal College)
Learning Objectives 
At the end of the course, participants will be able to:
  • Describe the steps required to successfully manage an adverse event immediately after it takes place
  • Develop a timeline for a moderately complex patient safety event
  • Gain skill in interviewing patient/family members and healthcare providers in the context of a systems review
  • Develop a SAFER Matrix as part of a systems review of a patient safety event
  • Use a completed SAFER Matrix to analyze information gathered while conducting a systems review
  • Write a system-focused issue statement that is succinct, uses non-judgmental language and identifies a single issue
  • Describe the important principles and practices for successful disclosure of harm to patients
  • Understand the information required for making decisions about an adverse event management plan and what components of it are needed to adequately address the needs of the patient, the healthcare provider, the healthcare organization and any regulatory agencies
  • Explain the concepts of ‘second harm’ and 'second victim' and how they can be avoided, minimized or recognized and dealt with through proper management of an adverse event
  • Describe the differences between reporting, informing and disclosing
Certificate Course Requirements
The Investigating and Managing Patient Safety Events Certificate will be granted upon successful:
  • Participation during the 5 days of face-to-face education (Introductory Investigating and Managing Patient Safety Events and Advanced Investigating and Managing Patient Safety Events courses)
  • Completion of a patient safety review including submission of related materials
  • Manage an adverse event immediately after it takes place
